HOW DO A PATIENT CAN SCHEDULE AN APPOINTMENT?
Patient can contact us directly at 281-823-8680 or click on appointment button for appointment request. Your health may require that you have a referral from your primary care physician before you can be seen and treated.  We will also need access to recent laboratory and imaging studies. All new patients are required to provide insurance information at the time of scheduling.

HOW DO A PATIENT REGISTER AS A NEW PATIENT?
Its easy. Once you contact US, you will receive emails and the second enabling you to fill out your new patient forms. The invitation will include instructions. This is a quick process. We will need your first name, last name, date of birth, zip code and e-mail address. To verify your identity, your information must match what we have in our system.

ONCE PATIENT ARRIVE
When patient arrive, our front desk personnel will check you in, review your information, provide you the necessary forms you need to complete, collect any payment due and notify the medical staff that you have arrived.
